Trafford Home Based Treatment Team (HBT)

About our service

The Trafford Home Based Treatment team (HBT) are a team of multi-professionals,supporting people who are currently in a mental health crisis or having a relapse of their existing mental illness and require intensive support. We provide a rapid assessment and treatment in their own homes or community venues, as an alternative to an inpatient admission. We also provide flexible support tailored to individuals’ needs which can include home visits up to three times a day to assess, listen and talk about difficulties currently being experienced..

Our team provides inreach to the inpatient unit and is able to support service users on extended leave from the wards. We also work closely with community teams to facilitate supported discharge, which is when a person is able to go home from hospital sooner than they usually would by increasing support at home in the initial period following inpatient discharge. We take referrals from both north and south Community Mental Health Teams (CMHT), Early Intervention Team (EIT), Trafford Extended Services (TES) and Rapid Assessment, Interface & Discharge service (RAID), with GP referrals going to CMHT as the single point of access.

Home Based Treatment is a 365 days a year service and covers the 24 hour period. Access to a qualified health professional is available 24 hours a day.

We can provide clinical and therapeutic interventions; 1 to 1 support, medication monitoring, daily planning and activities, anxiety management, benefits and housing advice, risk management and safety planning, sleep hygiene and much more. Each service user support by the team will receive a physical health check, this can be delivered in our own clinic at the Chapel Road Clinic or at home.

We engage carers of our service users to help provided and ensure that they have relevant contact information for services and advice on what to do in an emergency or times of concern. We offer assessment and support for all cares of Home Based treatment service users.

The team consists of Three consultant psychiatrists, a psychologist, mental health practitioners, occupational therapists and support workers.
