Carer Experience Survey
Please complete this survey if you care for someone accessing services at GMMH.
If you access services at GMMH we would love to hear what you think about our Trust. Whether your experience was good or bad, telling us about it can make a big difference to how we do things in the future.
Please take a moment to give us your feedback online. It is a simple and anonymous way to tell us what you think of the care or treatment you received in the NHS.
We have four experience questionnaires, which also incorporates the Friends and Family Test. Please complete the survey which applies to you (opens in a new window).

The NHS Friends & Family Test (FFT) is about giving patients and their carers the opportunity to provide quick feedback on their care and treatment experience.
You can say what is going well and what can be improved so that people who make decisions about local healthcare can take your views into account.
The Friends and Family information we collect is published nationally by NHS England (opens in a new window) and can also be found on NHS.UK (opens in a new window).

The GMMH Service User and Carer Experience Questionnaires allows you to give more in-depth feedback of your experience of GMMH’s services.
You can provide us with your views and your experiences of care, which are vital in helping us shape and improve our services.
It is a simple and anonymous way to tell us what you think of the care or treatment you received with us.
We have embedded the Friends and Family Test (FFT) question within this questionnaire, so you don’t need to complete two separate surveys.
You can contact our Customer Care Team to request a paper copy of the survey:
Telephone: 0800 587 4793 (freephone) or email: customercare@gmmh.nhs.uk
The information will give us invaluable feedback on what you think of the care and treatment you have received, which along with existing ways of gathering feedback, will help us to make improvements for our service users and carers.
We use feedback to inform staff induction, training and development, and we do our best to communicate with service users and carers about the improvements we have made, including displaying ‘You Said, We Did’ posters in services.
Some examples of comments from our service users and You Said, We Did are detailed below:
You Said - “Can we have better garden furniture and a new Xbox”
We Did - All items have been ordered and awaiting delivery.
You Said - "A lack of therapy and activities and not enough staff”
We Did – Ward Activity Coordinator is now working five days a week.
You Said, We Did – Following carer feedback about the entrance to Irwell Ward, the ward entrance has been revamped. A gentleman accessing support on the ward assisted with the development of a Feedback Board on the ward.
