Assessment Service

The Assessment Service provides an appropriate and professional mental health assessment service for people living in the borough of Bolton. 

The Assessment team receives screened referrals from the Single Point of Access and thereafter categorise referrals according to the following priorities:

  • Emergency face-to-face assessment, this usually takes place within 24 hours
  • Urgent face-to-face assessment, this usually takes place within 7 days
  • Routine face-to-face assessment,  this usually takes place within 28 days
  • GP telephone call for medication advice, this usually takes place the same day

The Assessment Service will provide an appropriate and professional face-to-face assessment of a service user’s level of risk and needs followed by partnership outcome planning with the service user (and carer where possible) to determine the best possible onward care pathway.

We will occasionally provide brief 1:1 follow-up sessions, if necessary, as part of the ongoing assessment in order to determine the best onward care pathway.
