Referring to the Neighbourhood Mental Health Services
Working in partnership with voluntary, community and social enterprise organisations and other partners, we provide Living Well services in Bolton, Manchester, Salford, Trafford and Wigan.
New name, same trusted care
On Monday 30 March 2026, Living Well services will change their name to Neighbourhood Mental Health Services (e.g. Bolton Living Well Service will become Bolton Neighbourhood Mental Health Service).
We’ve also refreshed the service identity – which includes things like the logo on the top of letters, leaflets and forms. This has been developed with people who have lived experience.
Most changes will be digital, meaning costs are minimal. A consistent name and identity across Greater Manchester will make it easier for everyone to recognise and understand who we are and what we do.
About the services
We run the services jointly with voluntary, community and social enterprise organisations and other partners.
We support adults (18+) registered with a GP in Bolton, Manchester, Salford, Trafford and Wigan, who are experiencing mental health issues.
Neighbourhood Mental Health Teams
The team supports people who are experiencing significant mental health difficulties that are having a serious impact on their life, where support from their GP, talking therapies, or community services hasn’t been enough. A diagnosis isn’t needed.
The team will support people’s mental health recovery and work with lots of partners to provide joined-up support for wider issues that may be affecting their mental health e.g. housing, finances, substance use, domestic abuse, physical health and more.
Who can refer
Referrals to the neighbourhood mental health team are accepted from:
- GPs
- Health, social care or voluntary, community and social enterprise professional
- Local authority services (e.g. housing)
- Police, probation, and criminal justice services
How to refer
Referrals should be sent via email to the relevant borough team as follows (please note - the new email addresses below will be live from 30 March 2026. If you need to refer before then, please use the current emails address, or contact the team at the details below).
To make sure people get the right support, the referral form must be used. It includes important information and asks for consent, which helps us understand the person’s needs and work with partners to provide the right support, without delay.
We need the individual’s consent to provide support; referrals should only be made once this consent has been obtained.
