My Health Passport
We are pleased to introduce My Health Passport, a new patient-owned document.
The passport has been designed to help people share important information about themselves with the professionals involved in their care.
It has been co-designed with service users, carers and professionals to ensure it is accessible and covers everything you would need it to.
My Health Passport gives you the opportunity to record information such as:
- What is important to you
- How you prefer to communicate
- People who are involved in your care and support
- What helps you feel safe and well
- Things that may cause distress or affect your wellbeing
- Preferences for treatment and support
- Physical health needs and other important information
Why use My Health Passport?
Many people tell us that having to repeat their story to different services can be frustrating, stressful and sometimes difficult. My Health Passport helps you:
- Share your story once and use it with different services
- Ensure professionals understand what matters most to you
- Support more personalised and trauma-informed care
- Improve communication between services
- Increase your choice and control in decisions about your care
The passport belongs to you. You can complete it yourself, complete it with support, and choose when and with whom you share it.

We are working on developing a course through our Recovery Academy that will support people to make the most out of completing the document. More information on that to follow.

Share your experience of using My Health Passport
We hope My Health Passport will help ensure your voice is heard and that the people supporting you understand what matters most to you.
