Community Mental Health Teams

Community mental health teams (CMHTs) provide multi-disciplinary assessment, treatment and care of individuals with severe and enduring mental health problems.  They provide a service to individuals from 16 years of age up to 70 years of age with a diagnosed functional mental health problem, unless the service users’ needs would be best met by the Older Adults CMHT or vice versa. This provision was delivered in partnership by GMMH and the Norther Care Alliance.
 
On 1 June 2026, responsibility for the social care element of the CMHT’s transferred to Salford City Council. Greater Manchester Mental Health Trust, the Northern Care Alliance, and the Council worked closely to ensure the transfer of services and employees was safe and smooth for all involved, with no disruption to the care people receive. Health related provision in the CMHT’s remains the responsibility of GMMH. GMMH will work in partnership with SCC to ensure that service users have a good quality experience. Social workers remain co located in the CMHT’s with GMMH staff.
